How do PillPal and Medisafe differ in approach?
Medisafe wants to be your medication hub: it knows your drugs, your doctor, your measurements and your caregiver, and it uses an account to keep all of that in sync. PillPal wants to be the thing that makes the dose happen and remembered, with the least possible surface: one Today screen, a reminder that stays until you act, a widget, a streak. Neither is wrong. They are for different amounts of medication in your life.
Setup and daily use
PillPal opens with a short questionnaire and has you adding your first pill in under a minute, with no email and no password. Medisafe asks for an account (or a guest mode with limits) and walks through a longer profile. Day to day, both put Taken and Skip on the notification; PillPal adds a ten-minute Snooze and marks reminders time-sensitive, so they get through Focus unless you switch time-sensitive notifications off. Time-sensitive is not an alarm, though: on a muted iPhone a PillPal reminder arrives silently.
Privacy and offline
PillPal stores pills, schedules and history on the device and works with no internet; there is nothing to log into, and the little that does leave the phone (anonymous analytics, purchase status) is listed in the privacy policy. Medisafe syncs to its cloud so caregivers and reports can work, which is the trade-off for those features. PillPal's privacy policy spells out the little that does leave the phone.
Which one is better for caregivers?
If you need a family member alerted when a dose is missed, refill tracking, or interaction warnings, Medisafe has them and PillPal does not. Medisafe's Google Play listing names its caregiver feature Medfriend and notes that the drug-to-drug interaction checker works only in the US, with the app set to English. PillPal's caregiver story is simpler: set it up on the other person's phone in five minutes, share the plan, and review the dose history together. How that works.

A reasonable path is to start with the simple one and move up only if you find you need the hub. Interaction warnings are a question for your pharmacist either way, not for a reminder app.

Can PillPal notify a family member when I miss a dose?
No. PillPal has no accounts and no cloud sync, so there is no channel to send a remote alert over.
